■ 논문소개

이번에는 ‘Effects of Shakuyaku-Kanzo-to on Extrapyramidal Symptoms During Antipsychotic Treatment’라는 제목을 가진 논문을 소개하고자 한다. 향정신병약물(antipsychotics) 복용 중 초래될 수 있는 추체외로증후군(extrapyramidal symptoms)에 작약감초탕의 효과를 확인한 논문이다. 

추체로가 수의적인 운동을 담당하는 반면, 추체외로는 근육의 긴장ㆍ신체의 안정ㆍ운동의 조화 등 불수의적으로 행해지는 여러 가지 운동을 담당한다. 추체외로에 이상이 발생하면 근긴장이상, 정좌불능증, 파킨슨증후군, 서동증 및 진전 등의 증상이 나타날 수 있는데 이를 총칭하여 추체외로증후군이라고 부른다. 

향정신병약을 복용하는 환자 중 일부에서 이러한 추체외로증후군이 발생하게 되는데, 이러한 경우 항콜린성 약물이 흔히 치료제로 선택된다. 하지만 항콜린성 약물은 입마름ㆍ졸림ㆍ인지기능저하ㆍ변비ㆍ배뇨곤란 등 이차적인 부작용이 있을 수 있으며 특히 고령 환자에서 빈발하는 경향을 보인다. 상기한 배경을 토대로 이 논문에서는 항콜린성 부작용이 없는 작약감초탕이 향정신병약물 복용 후 나타난 추체외로증후군에 치료 효과가 있는지를 확인하였다.

[환자/대상]
이 임상시험은 향정신병약물 복용하고 있으나 파킨슨병 진단을 받지 않은 정신과 질환자를 대상으로 하였다. Inclusion criteria는 Drug-Induced Extrapyramidal Symptom Scale(DIEPSS) 2점이상이고 적어도 한가지 항목에서 2점이상을 받은 환자를 대상으로 하였다. 기질성 뇌질환이나 알콜이나 약물 중독환자는 제외하였다. 

[평가항목]
정신증상은 Positive and Negative Syndrome Scale(PANSS)와 Clinical Global Impression(CGI) scale로 평가하였고, 추체외로증상은 DIEPSS 와 Barnes Akathisia Rating Scale(BARS)로 평가하였다. 그 이외에도 혈중 HVA(도파민 대사체)와 프로락틴 농도도 측정하였다.

[연구 디자인/방법]
이 연구에는 총 22명(여성이 21명이다)의 추체외로증후군을 가진 정신 질환 환자들이 포함되었는데, 작약감초탕군과 biperiden투여군으로 무작위로 배당되어 2주간 처방받았다. 작약감초탕은 하루 7.5g 투약하였으며, biperiden은 하루 3mg 투약하였다. 따로 복용중인 약들은 계속 복용하였다.

[결과]
도중에 2명이 탈락하여 총 20명이 임상시험을 마쳤다. DIEPSS 증상은 두 군 모두 현저한 향상을 보여주었다.(P<0.01) 하지만 PANSS, CGI scale, BARS, plasma HVA level, serum PRL level 등에서는 두 군 모두 유의한 차이를 보여주지 못했다.

ANOVA 통계방법상으로 DIEPSS total score는 time×drug 상호작용에서 유의한 변화를 보여주지 못했다. 하지만 DIEPSS total score에서 time effect는 유의한 효과를 나타냈다.(P<0.01) DIEPSS의 subscale score에서는 작약감초탕그룹에서는 dystonia 항목에서 time×drug 상호작용에서 유의한 결과가 나타났으며(P=0.0059), biperiden그룹에서는 bradykinesia 항목에서 유의한 결과를 나타냈다(P=0.0131)

■ 해설

이번 논문은 향정신병약을 복용할 경우 흔하게 나타나는 추체외로증후군에 작약감초탕이 얼마나 효과가 있느냐에 대한 임상연구이다. 추체외로증후군에는 biperiden과 같은 항콜린제를 처방하는데, 항콜린제와 효능이 유사하다고 생각되는 작약감초탕이 추체외로증후군에 효과가 있는지 확인해보았으며, 실제로 임상시험을 거쳐보니 효과가 있었다는 논문이다. 작약감초탕은 추체외로증후군의 증상 중에서도 dystonia(근긴장이상증) 항목에서 상당히 유의한 효과를 나타내었다.

이번 논문을 소개하는 이유는 다음의 두 가지 이유에서였다. 첫째 임상을 하다보면 특히 고령의 환자의 경우 향정신병약을 복용하고 나타나는 추체외로증후군은 아닐지라도, 근긴장이상증이나 진전 파킨슨병 같은 질병은 흔히 접하게 된다. 이런 경우 작약감초탕을 처방해볼 수 있다. 둘째 특히 일본에서는 양방에서 항콜린제를 처방하는 질환들에 작약감초탕이나 유사처방을 많이 활용하고 있다는 사실이다.(보험한약 임상사례 47편 참고) 부교감신경이 흥분하고 평활근이 과도하게 수축해서 생기는 질환들은 무수히 많다. 그러므로 근육긴장이상 복통 과민성장증후군 알레르기비염 과민성방광 등 항콜린제를 처방하는 질환들에는 작약감초탕을 적절하게 활용해볼 수 있을 것이다.

A Brief Description of the DIEPSS Subscales 
The severity of each item is rated from 0 (normal) to 4 (severe). 

1. Gait(보행) 
Shuffling, slow gait. Evaluate the degree of reduction in speed and step, decrease in pendular arm movement, stooped posture and propulsion phenomenon. 

2. Bradykinesia(운동느림) 
Slowness and poverty of movements: Delay and/or difficulty in initiating and/or terminating movements. Rate degree of poverty of facial expression (mask-like face) and monotonous, slurred speech, as well. 

3. Sialorrhea(침흘리기) 
Excess salivation. 

4. Muscle rigidity(근육강직) 
Resistance to flexion and extension of upper arms. Rate cogwheeling, waxy flexibility, lead-pipe rigidity and the degree of flexibility of wrists, as well. 

5. Tremor(떨림) 
Repetitive, regular (4-8 Hz) and rhythmic movements observed in the oral region, fingers, extremities and trunk. 

6. Akathisia(좌불안석증) 
Subjective inner restlessness and related distress; awareness of the inability to remain seated, restless legs, fidgety feelings, desire to move constantly, etc. Rate increased motor phenomena (body rocking, shifting from foot to foot, stamping in place, crossing and uncrossing legs, pacing around, etc.), as well. 

7. Dystonia(근긴장이상증) 
Symptoms induced by the hypertonic state of muscles. Stiffness, twisting, and persistent abnormal position of muscles observed in tongue, neck, extremities, trunk, etc. Rate tongue protrusion, torticollis, retrocollis, trismus, oculogyric crisis, Pisa syndrome, etc. 

8. Dyskinesia(운동이상증) 
Hyperkinetic abnormal movements. Apparently purposeless, irregular, and involuntary movements observed in face, mouth, tongue, extremities and/or trunk. Include choreic and athetoid movements, but do not rate tremor. 

9. Overall severity 
Overall severity of extrapyramidal symptoms.
